My initials indicate I have accepted the following terms:
___ I agree to pay the tuition of $100.00 by the first class day of each month.
___ I understand that the full fee of $100.00 is due each month, regardless of whether my child is absent from preschool for any reason.
___ I give permission for Allison Bartolo to administer first aid or obtain emergency medical treatment for my child in case of emergency and agree to pay any expense incurred.
___ I understand that Little Star begins at 9:00 am and ends at 12:00 pm and agree to drop off and pick up my child at the scheduled times.
___ I understand that there will be a $5.00 early drop off / late pick up fee. If a child is dropped off or picked up more than 5 minutes late, a $5.00 fee will be added t the student's bill.
___ I understand that if my child has flu or cold like symptoms such as diarrhea, vomiting or a runny nose, my child will not be allowed to attend preschool until symptoms are gone for a 24 hour period. If my child has a fever, my child will not be allowed to attend preschool. (The fever must be gone for at least 24 hours.)
___ My child's immunizations are current, and will be kept current for the duration of their enrollment in Little Star Preschool.
___ My child is toilet trained.
___ I agree that I am responsible for any damages to Allison Bartolo's real/personal property accidentally caused by my son/daughter.
___ I agree to hold harmless Allison Bartolo from any liability for injuries incurred to my son/daughter during Little Star.
___ I understand that I should not allow my son/daughter to bring toys or other articles from home for play, as there is a possibility they could be lost or broken - exception would be sharing days. I also understand that food to share should only be brought on my assigned snack days.
___ I understand that after my child is enrolled in the Little Star program, there will be a two to four week adjustment period. This is to ensure the child will be compatible with or ready for the Little Star preschool program. After the two to four week period, if the child is not compatible with or ready for preschool, I understand I will receive a two week termination notice.
___ I have read and understand the items listed under "Little Star Tidbits".
___ I give permission for my child's name, address, and telephone number to be included on a class list.
___ If the school district decides that the weather is bad enough to cancel OR delay school, for safety reasons we will NOT hold preschool on that day. Make up days will be scheduled and announced as needed. If the weather is bad, please check the news or the Internet for information.
___ Note from teacher * If I am ill to the point where I am unable to run preschool or in case of an emergency, I will make every attempt to give you enough notice so you may make other arrangements for your child. My sick days as well as inclement weather days will be rescheduled as make up days.*
